Guidelines

1. The Infant Dedication (ID) Form may be obtained from the Church office or Church’s website.

2. A parent should complete and submit form to the Church Office.

3. The child's original birth certificate must be presented along with the completed  ID form to the Church office staff

4. A copy of the birth certificate will be retained and the original birth certificate returned to the parent.

5. The office will advise the parent of the decision re the request within 5 working days of the submission of the required documents.

6. The Church Sponsor (a BBC member) will be assigned to parents who are not members.

7. The Church Sponsor is responsible to follow up with the parents regarding the child’s spiritual development.
